Who will help you with the Capacity and Access plan, as well as other areas of the DES. Ensuring you telephony system is up to date , and information is gathered , so you can provide to NHS England and your local ICB.

They will support you with the enhanced access system,  and  help promote the NHS App. Assist in the delivery, adoption and improvement of new technology to enhance patient access and experience. Build relationships and facilitate the collaboration between Practices to support the delivery of care to all patients registered with the PCN Practices, where Hub arrangements are in place.

They will review and improve the PCNs digital maturity with the use of data, ensuring data quality is improving. Help develop strategies to move the PCN along the Maturity Matrix over the coming years.

To understand demand, capacity and activity with the use of data and drive improvements.

Support in the patient experience of access and staff experience at work and ensure operational efficiency.
